I accept MediCare insurance for payment, and do the paper work for you. There may be deductibles and co-payments
that will be your responsibility. I will bill your secondary insurance automatically. Please bring your MediCare
and insurance cards to your first visit.
For all other insurance I fill out a "superbill" for you to submit to your insurance company. You will be reimbursed
if you have a PPO or open plan, but not if you have an HMO or Kaiser. My services are billed as any licensed physician
services. I expect payment at the time of visit by check or cash. If you encounter trouble with reimbursement
my office will assist you.
For those who cannot submit a claim to any insurance company, and who cannot afford my services there is a sliding fee
scale.
I do not accept new patients under Work Comp coverage.
